All funding requests of the WMS PTA should be completed on this form.
IRS REGLUATIONS STIPULATE THAT ALL FUNDING REQUESTS THAT ARE APPROVED BY THE WMS PTA MUST BE ACCOMPANIED BY A RECEIPT(S) IN ORDER TO BE PAID. PLEASE SAVE AND TURN IN YOUR RECEIPTS!
Any requests made less than one week prior to a PTA meeting will be discussed at the following PTA meeting. When the 'Submit' button is clicked, a copy of the form will be sent to Mr. Berzinski and the PTA at
